74 year old female who had left breast reconstruction 20 years ago with a silicone implant presented with grade 4 capsular contracture — painful hardening and distortion of the capsule around her left breast implant.

She underwent a left breast capsulectomy, implant replacement with moderate profile 659 ml silicone, round, smooth implant, as well as a right mastopexy. She is very happy that she is able to fit into a normal bra now.
